Rectangular wooden plaque with silver painted rim and laminated plastic face. The face of the plaque has a thick silver and black border. Sealed under the plastic is a cream rectangular piece of paper. It shows an illustration by Sandy Dvore of David Bowie in his Ziggy Stardust era, singing into a microphone. The illustration is in a loose, sketchy style, in pencil. Under this, is 'CMA', also illustrated in pencil, in stylised block lettering. There is other smaller text at the top and bottom of the plaque face (detailed in Inscriptions). \r\n\r\nThe back of the plaque has a rectangular metal hanger hook at the top, with a serrated edge.
